A Cary dentist has had her license revoked after authorities determined she did not properly monitor a patient who died while she extracted a tooth.

A Cary dentist can never practice in North Carolina again after a patient died in her care. Catherine Thoits, 57, a patient of Dr. Toni Mascherin died while under sedation.
